By sending this electronic form to Music Rhapsody, I, as the parent, Agency Representative, or Legal Guardian for the child designated in this form, hereby hold Music Rhapsody harmless for any and all damages, injuries or losses that may be sustained while in the care of Music Rhapsody.

MUSIC RHAPSODY TUITION POLICIES

Music Rhapsody classes run continuous from September thrugh June. Students may join at any time during the school year and tuition will be prorated (paying full tuition and other fees for the first six weeks). We must receive your completed registration form, first tuition payment, and materials fee before your child's first class.

• There are no refunds for missed classes. Each student is allowed ONE makeup class within their six weeks, which must be arranged through the office.

• If you drop a class during the school year, but later decide to re-enroll, a $10 re-enrollment fee will be added to your tuition payment.

• If you are unable to complete the school year you must notify the office (in writing, by mail, or by email at info@musicrhapsody.com).

• A $25 fee will apply to returned checks.

• If you request a switch in class times you will be charged a $10 switch fee due to extra office costs.

• Tuition paid is non-refundable and cannot be applied to any other session.
